What happens after you send the form?
We'll send an email to the address you give us with a secure link to our booking system. Open it and choose a time that suits you. If the email hasn't arrived within a few minutes, check your junk folder or call us and we'll book you in over the phone.
New patients can bring their Medicare card and health fund card, if they have them, and a list of any medicines they take. If you're booking for a child, tell us their age so we can allow the right amount of time.
